Accessing your health record

Your health record includes information like any conditions or allergies you have and any medicine you’re taking.

Most patients will automatically have access to more information added to their health record. This includes letters, test results and appointment notes.

You can use your NHS account (through the NHS website or NHS App) or Patient Access to access your health record.

Your online health record will show only from the date you registered with the surgery, or from when online record sharing went live in December 2022 (whichever is most recent). You’ll need to ask the surgery for online access to your full health record if you require older information.

Request access to your full health record online

If you would like to request a longer period, or full electronic health record please complete this form and return it to the surgery with a copy of your photo ID. This will then go to your named GP for approval.

Subject Access Requests (SAR)

If you need to request a specific portion of your medical record, or the record in its entirety, including your paper “LG” notes, you can put in a Subject Access Request (SAR). To do this you will need to complete a SAR Form and hand it in to reception with a copy of your photo ID. SARs can take up to 28 days to process. Where possible we will provide these records to you in an electronic format.

Access to other medical records

For any other medical records, you need to ask for them at the NHS service you went to. This includes records like your hospital record.
