Self referrals
There are a number of services locally that you are able to make self referrals to.

- Youth counselling
- Weight management
GP referrals
To request a GP referral:
- phone us on Burford Surgery 01993 822 176 or Carterton Health Centre 01993 822 176 or visit the surgery
Provide as much information as you can as this will help us to process your request.
Tracking a referral
To track an existing referral:
- Phone us on (01993) 822176 (option 2) and speak to a member of our Referrals team. There is an answerphone on this line. Please leave a message, if the Secretaries are not available, and one of them will get back to you.
- Use Your NHS App to check and manage active referrals, hospital appointments and any hospital waiting lists you have been added to.
